To lock two pages together so that they cannot be separated, right click on the thumbnail of one of the pages along the bottom of BookSmart and select Page Lock Options, then Lock Spread Together. Now no matter if you add or remove pages, these two pages will remain locked together so that you have the correct left and right pages for this particular spread.
